Win a Fitbit Blaze -  HOW TO ENTER: The 2016 Get Heart Smart Win one of 3 Fitbit Blaze smartwatches commences on August 1, 2016 and ends October 31, 2016. Entries must be submitted by 11:59 p.m. EST on October 31, 2016 ("the Contest Closing Date"). To enter, go online to Get-heart-smart.hrinz.org, completely fill out the contest entry form, including your daytime phone number, and click on the Submit button. Limit of one (1) entry per individual. No purchase or donation is necessary to enter or win. All entries become the sole property of the Sponsor and none will be returned for any reason. Entries will be declared invalid if they are late, illegible, incomplete, damaged, forged, or otherwise not in compliance with these terms and conditions. CONTEST DRAW: Three (3) winners will be selected from among all eligible entries received in a random draw conducted at the Jack Media Sydney Office, 68-72 Commonwealth Street Surry Hills, NSW, Australia, at 10am on November 14, 2016 ("the Draw Date").
